WECAN has been commissioned by West Essex CCG to support the discharge of mental health patients back to their homes

A new short-run pilot coordinated by WECAN across West Essex will see help and support provided to patients as they prepare to return home. Teams in each of the three districts – Harlow, Epping Forest and Uttlesford – will ensure that homes are safe and ready to return to, from basic tasks such as cleaning the home after a period of absence, through to home improvements to facilitate the provision of care – such as the installation of key safes.

Outcomes of each intervention will be assessed, and the value of the trial reviewed after the initial period, with an aim to upscaling the scheme.
